About this Course

2023 Camp Emergency Forms will be collected digitally, once per year. A camp emergency form must be completed in full for any child registering in an Esquimalt Recreation camp, including spring break camps, summer camps, winter break camps AND pro-d days. If your child requires extra support, you MUST fill out an Inclusion Intake Form 2023 and submit it to OSC@esquimalt.ca to confirm your child's registration. Please have your child's Care Card # and list of guardians and alternate contacts phone #s available to complete this form. *** Please complete all fields, select NEXT, select Checkout to complete your form.
